Thomas Dale Forrest

Thomas Forrest, age 66, passed away on Friday, May 31, 2024, at Jackson Madison County General Hospital. 

He was born on May 21, 1957, in Hammond, Indiana to Marie Holland Mallard and the late Edward Forrest.

In addition to his father, Thomas was preceded in death by one grandchild, Macon Forrest.

He is survived by his wife, Tina Readenovr Forrest of Humboldt; mother, Marie Holland Mallard of Atwood; four sons, Collin Jackson (Linda) of Nevada, Shannon Forrest (Jessica) of Lexington, Thomas Forrest of Trenton and James Forrest (Amanda) of Trenton; one daughter, Amber Worrell (Matt) of Trenton; two sisters, Vicky Hurley (Bear) of Atwood and Tammy Readenovr of Gleason; and nine grandchildren.

A memorial gathering will be held from 1:00 P.M. until 3:00 P.M. on Monday, June 10, 2024, in the chapel of Medina Funeral Home.
