He was born on September 3, 1971 in Palatine, Illinois to the late Billi Jean Pace Howard and David Allen Howard.
Survivors include his father, David; two brothers, Richard Howard and Cory (Judy) Howard; one sister, Kathy Howard and two nephews Alexander and Logan Howard.
Other survivors include multiple aunts and uncles.
Graveside service for Mr. Chadwick will be conducted at 2:00 p.m. on Tuesday, June 11, 2019 in Lexington City Cemetery.
